How to use these files

A password is required to open some of these documents. The password is open.

Documents 6 through 13 are Microsoft® Word97 templates. You must use Microsoft® Word97 or Microsoft® Word2000 to fill out and print the application forms. To determine if you have the correct version, load Microsoft® Word, click on "Help" from the toolbar and then select About Microsoft® Word. The first line on this panel will identify the version of Microsoft® Word. The only acceptable versions of Microsoft® Word are Microsoft® Word97, a standalone product or a member of the suite of products in MS Office 97 Standard or Professional versions, or Microsoft® Word2000, a standalone product or a member of the suite of products in MS Office 2000 Standard or Professional versions.

The templates contain form fields that permit the entry of requested information. Only the entered text that you actually see within each field is the text that will print. If a field is not large enough to display your response, attach a separate sheet or sheets, referencing the form and the question number and part. Some of the form fields have the ability to accept two lines of information within the form field.

Follow these guidelines when filling out these forms:

• Do not use all upper case characters; use upper and lower case characters.
• Use abbreviations when possible (e.g., "St" for "Street"; "Blvd" for "Boulevard").
• Use a standard 2-character abbreviation for each state entered (e.g., "OH" for "Ohio").
• Use numeric dates only (e.g., 10/01/1997 or 10/1997).

Other requirements

Before being admitted to the practice of law in Ohio by examination, an applicant must take and pass the Multistate Professional Responsibility Examination (MPRE). An applicant may take the MPRE at any time before or after taking the Ohio bar examination. Applicants should make arrangements for taking the MPRE directly with the National Conference of Bar Examiners. Applications to take the MPRE are available from either of the following:

Filing your completed application

After completing the forms on this web page, print the forms, and then execute the documents. Please note that your signature on some of the documents must be notarized. Only executed originals will be accepted for filing; electronic filings are not acceptable.
